How the AI Risk Register grades each entry: six weighted governance fields and an accepted-high-residual gate that forces UNGOVERNED when a high-residual accepted risk has no named owner — which is how entry R-03 scores 78 and still fails.
How the gate works
This tool grades whether each entry in your AI risk register is governed enough to be defensible. Each control is marked from your own evidence and weighted into a 0–100 score — and then the score is run past a gate that can only make the verdict worse.

The verdict bands
A risk you accept, at high residual severity, with no named owner (or an overdue review) is UNGOVERNED — regardless of the score.
In the worked sample it scores 78 out of 100 and still reads UNGOVERNED — Entry R-03 is an accepted, high-residual risk that scores well on paper — but nobody owns it, so the register isn't defensible. The one thing to fix first: Name an owner for the accepted high-residual risk.

Can something score well on the AI Risk Register & Treatment Tracking and still fail?

What is a worsen-only gate?
A dispositive rule that can only lower a verdict, never raise it. One fatal flaw overrides an otherwise-good score, because a high average can hide a single disqualifying gap. The tool also names the one thing to fix first — here, "Name an owner for the accepted high-residual risk".
