Tool Gate · How it works
How the PM Deferral & Pencil-Whip Tripwire gate works
How the PM Deferral & Pencil-Whip Tripwire scores one asset's maintenance execution: a critical-asset gate forces RUN-TO-FAILURE when logged time can't cover the work — so a 78/100 asset showing 100% completion is really being pencil-whipped.
How the gate works
This tool grades one asset's PM execution against pencil-whipping and repeat deferrals. Each control is marked from your own evidence and weighted into a 0–100 score — and then the score is run past a gate that can only make the verdict worse.
The 3 weighted controls
| Signal | Weight |
|---|---|
| Completion compliance | 45 |
| Deferral integrity | 30 |
| Time plausibility | 25 |
| Total | 100 |
The verdict bands
On a criticality-A asset (line-down / safety on failure), a PM deferred twice or an implausible completion time — logged minutes far below the standard the work should take — forces RUN-TO-FAILURE regardless of the execution score. For that asset the compliance percentage is a lie; a signature is not a maintenance event, and the gate only ever holds an asset back.
In the worked sample it scores 78 out of 100 and still reads RUN-TO-FAILURE — This criticality-A asset shows 100% completion and a clean deferral record — the execution score is 78, which alone reads RELIABLE. But the plausibility signal is 0.13: a 40-minute standard PM logged in about 5 minutes. Logged time can't cover the work, so the pencil-whip gate forces RUN-TO-FAILURE. The compliance number is a lie for this asset. The one thing to fix first: Time plausibility — a 40-minute standard logged in ~5 minutes (0.13) can't have been performed. Investigate the pencil-whipping on this A-asset before trusting its 100% compliance; log plausible times or reopen the PM and it releases..
Common questions
Can something score well on the How the PM Deferral & Pencil-Whip Tripwire works and still fail?
Yes. The weighted score is run past a worsen-only gate — the Critical-asset integrity — that can only make the verdict worse. In the worked sample it scores 78 out of 100 and still reads RUN-TO-FAILURE, because this criticality-A asset shows 100% completion and a clean deferral record — the execution score is 78, which alone reads RELIABLE. But the plausibility signal is 0.13: a 40-minute standard PM logged in about 5 minutes. Logged time can't cover the work, so the pencil-whip gate forces RUN-TO-FAILURE. The compliance number is a lie for this asset.
What is a worsen-only gate?
A dispositive rule that can only lower a verdict, never raise it. One fatal flaw overrides an otherwise-good score, because a single disqualifying gap shouldn't hide behind a high average. The tool also names the one thing to fix first — here, "Time plausibility — a 40-minute standard logged in ~5 minutes (0.13) can't have been performed. Investigate the pencil-whipping on this A-asset before trusting its 100% compliance; log plausible times or reopen the PM and it releases.".
Scores an asset's maintenance execution, never a person; a flag points at a record to check. Deterministic and offline on your own CMMS data, with no benchmark multiplier. Not a safety-compliance certification and not engineering advice.
