How the NCR-to-CAPA Routing Triage gate works

This tool grades every NCR's actual CAPA call against the routing verdict it should have gotten. It reads 8 NCRs and rolls them into one verdict — the worst, never the average — then a gate that can only make it worse.

An NCR whose severity or REPEAT OFFENDER status routes it to OPEN A CAPA but has no CAPA opened is a MISSED ESCALATION - and even one missed escalation overrides the CAPA-load read, however low the overkill share, so the log reads ESCALATIONS MISSED.

ESCALATIONS MISSED

Why: Four of twelve NCRs are CAPA overkill - routine first-occurrence scratches that earned a CAPA anyway - which alone would only read DROWNING IN MINOR CAPAS at 57% of open CAPAs. But two NCRs needed escalation and didn't get it: the third scratch on P-2201, a REPEAT OFFENDER, and an uncontained major dimensional defect. One missed escalation overrides the load read, so the verdict is ESCALATIONS MISSED. 2 of 12 NCRs missed escalation · 57% of open CAPAs are overkill · worst-not-average.

Fix firstNCR-2440 - the third scratch on P-2201 in 90 days is a REPEAT OFFENDER that was disposed instead of escalated; open its CAPA first, then fix-first moves to NCR-2448, the uncontained major.

No single NCR is the crisis — the clustering is. The roll-up takes the worst, the gate escalates when trouble compounds, and it names the one thing to fix first. Grades routing consistency against the log you provide; it is not a quality-system certification and does not open or close CAPAs for you.
