How the AI Incident Postmortem gate works
This tool grades an AI-incident postmortem before you close it. It scores 6 weighted controls into a 0–100 number — then a gate that can only make the verdict worse.
The 6 weighted controls
Any open critical action item forces NOT CLOSEABLE no matter how complete the writeup is. An incident closes when the critical fixes are done, not when the document reads well, regardless of the score.
Why: The postmortem is a perfect 100 — every section complete, root cause found, blameless framing. But one critical action item is still open, so the gate holds it NOT CLOSEABLE. An incident closes when the fix is done, not when the writeup reads well.
