How the Website Lead-Leak & Form-Failure gate works

This tool grades one form's likelihood of actually delivering the leads it captures. Each signal is scored 0–1; the verdict is the weakest one — then a gate that can only make it worse.

A form with no verified delivery path is a BLACK HOLE no matter how good the other five signals are - a lead nobody receives didn't happen. Delivery has to be independently confirmed with a real test submission that lands somewhere monitored, never assumed from a form that 'looks like it works.'
