How the Website Lead-Leak & Form-Failure gate works
This tool grades one form's likelihood of actually delivering the leads it captures. Each signal is scored 0–1; the verdict is the weakest one — then a gate that can only make it worse.
The 6 signals
A form with no verified delivery path is a BLACK HOLE no matter how good the other five signals are - a lead nobody receives didn't happen. Delivery has to be independently confirmed with a real test submission that lands somewhere monitored, never assumed from a form that 'looks like it works.'
Why: Five of six signals are clean - the success path, spam filtering, mobile usability, tracking, and notification speed all score 2. But delivery and notification is unverified (0), and that signal is the gate: a form nobody's confirmed actually delivers is a BLACK HOLE regardless of how good the other five look.
A clean sheet didn’t save it. The verdict is the weakest signal — and a top trigger is dispositive on its own — so it names the one thing to fix first. Grades the plumbing you test yourself - marks come from a real submitted lead, not an assumed or invented benchmark. Doesn't audit legal consent language on the form.
